Individual Healthcare Plans Tool

Individual Healthcare Plans (IHPs) are stored electronically as part of the student health record. The IHP wizard leads users through the creation process of customizing professional care plans to promote consistency of student healthcare and the ability to share them with others. Templates may be customized with any health condition allowing users to document assessment results, nursing diagnoses, interventions, and goals. As students transfer to different schools, IHPs track student progress overtime retaining a full history for review. In addition, providers have access to PSNI’s IHP Pavilion, an online database containing hundreds of professional healthcare plans written by school health providers worldwide. IHPs can be downloaded directly into SNAP Health Center with a click of a button and further customized if desired.
